Required Action: Medical Health Certificate

The deadline to submit students' Medical Health Certificates is September 14, 2026.

Lincoln families who have not yet submitted their child's Medical Health Certificate (signed and completed by a physician licensed in Argentina), must do so as soon as possible. 

If a student does not have a valid Medical Health Certificate submitted by September 14th, they will no longer be able to participate in physical activities, after-school physical activities, competitions, and/or field trips until a valid form is submitted to the Wellness Center. 

The form presented must be dated in the months of June, July, or August of 2026 to make sure that a student's forms are valid during the entirety of the 2026/27 school year at Lincoln. 

This was detailed in the Educational Contract package sent out to all Lincoln families in June of this year. 

If you have already sent in your child's Medical Health Certificate, please disregard this reminder.
